Management of Diffuse Pontine Gliomas in Children: Recent Developments

The prognosis for children with diffuse intrinsic
pontine gliomas (DIPGs) is dismal. Although DIPGs
constitute only 10–15 % of all pediatric brain tumors, they
are the main cause of death in this group with a median
survival of less than 12 months. Standard therapy involves
radiotherapy, which produces transient neurologic
improvement. Despite several clinical trials having been
conducted, including trials on targeted agents to assess
their efficacy, there is no clear improvement in prognosis.
However, knowledge of DIPG biology is increasing,
mainly as a result of research using biopsy and autopsy
samples. In this review, we discuss recent studies in which
systemic therapy was administered prior to, concomitantly
with, or after radiotherapy. The discussion also includes
novel therapeutic options in DIPG. Continuing multimodal
and multitargeted therapies might lead to an improvement
in the dismal prognosis of the disease.

Astrocytic tumors account for 42% of childhood brain tumors, arising in all anatomical regions and associated with neurofibromatosis type 1 (NF1) in 15%.
Anatomical site determines the degree and risk of resectability; the more complete resection, the better the survival rates. New biological markers and modern radiotherapy techniques are altering the risk assessments of clinical decisions for tumor resection and biopsy. The increasingly distinct pediatric neuro-oncology multidisciplinary team (PNMDT) is developing a distinct evidence base.

Background.
We sought to determine the surgical treatmentand functional outcome and identify the predictors of survival in a retrospective cohort of patients with spinal cord ependymoma using data collected from the Korea Spinal Oncology Research Group database.

Methods.
The data regarding 88 patients who had been surgically treated for histologically confirmed spinal cord intramedullary and extramedullary ependymoma from January 1989 to December 2009 were retrospectively reviewed.

Results.
Histopathological examination revealed myxopapillary ependymoma in 24 patients, ependymoma in 61 patients, and anaplastic ependymoma in 3 patients.

Gross total removal was achieved in 72 patients, subtotal removal in 15 patients, and partial removal in 1 patient. Twenty patients were treated with postoperative radiation. Fifty-two patients had stable or improved postoperative neurological function, while 36 experienced neurological deterioration. A permanent decrease in McCormick classification grade was seen in 17 patients. The progression-free survival rate was 87% for all patients at 5 years and 80% at 10 years. During follow-up, local recurrence/progression was seen in 13 patients. Diffuse meningeal spread developed in 2 anaplastic ependymoma patients. Postoperative radiotherapy after incomplete resection did not significantly correlate with longer times to recurrence. Multivariate analysis revealed histology and surgical extent of resection as independent predictors of longer progressionfree survival.

Conclusions.
Gross total removal alone is a good treatment strategy for spinal ependymomas. Early diagnosis and surgery, before severe paralysis, are important to obtain good functional outcomes. Subtotal resection with radiation therapy for intramedullary lesions appears to offer no advantages over gross total removal.

Although the World Health Organization (WHO) categorizes spinal ependymomas into three histological grades, difference in surgical outcomes between WHO grades I and II tumors are unclear. For these benign tumors, prognosis may be best determined by factors other than tumor grade alone, such as extent of resection. To analyze the effects of the extent of resection on different grades of spinal ependymomas, we performed a comprehensive literature review to identify adult spinal ependymoma patients who received surgical resection with a clearly identifiable WHO grade. A total of 175 patients were identified.
While grade III tumors carried the worst prognosis as expected (p < 0.001), grade I and II tumors did not differ significantly in outcomes following surgery. Overall, gross total resection (GTR, 68.7%, 114/166) provided significantly improved progression-free survival (PFS, p < 0.001) and overall survival (OS, p = 0.022) compared to the subtotal resection group. Surprisingly, the highest GTR rate was achieved for grade II tumors (78.8%, 78/99; p < 0.001) followed by grade I (58.9%, 33/56) and grade III tumors (27.3%, 3/11). Interestingly, PFS was significantly improved by GTR for grade II tumors (p < 0.001), but not for grade I (p = 0.705). Similar trends, although not statistically significant, were found for OS. Our results show that while GTR provides the best overall outcomes, GTR is most effective for classic grade II ependymomas, but not for grade I ependymomas. Despite having a lower WHO grade, myxopapillary ependymomas have a lower GTR rate, and benefit less from GTR.

Background: Tumor genesis of many pediatric malignancies remains
unclear. Data of immune function are lacking at diagnosis.
We prospectively analyzed 109 pediatric patients with malignancy
at diagnosis.
Methods: Lymphocyte subpopulations were characterized by
FACS, TREC-assay, and Immunoscope, cytokines by FACS and
ELISA.
Observations: We detected higher values of CD4+ T cells and
consecutively shifted CD4+/CD8+ ratio in all patients compared
with the control group. In patients with lymphoma, interleukin-2
was upregulated in all subpopulations.
Conclusions: On the basis of these findings an altered immune
function could be found in children with different malignancies at
diagnosis. Further investigations are necessary to identify tumorrelated
immune deficiency for novel therapeutic approaches.

Alterations in BRAF have been discovered in most pediatric lowgrade gliomas. Because the field has moved quickly during the past few years, there is not yet widespread awareness about what B-Raf normally does, how the BRAF gene is modified in gliomas, why mutant proteins promote gliomagenesis, and what an abnormal BRAF result means for diagnosis, prognosis, and treatment. Depending on the data from ongoing clinical trials, however, BRAF mutation screening could quickly become mandatory for all pediatric gliomas and perhaps even a subset of adult gliomas. Herein, these topics and different methods of testing for BRAF fusions and V600E point mutations are reviewed.

Purpose Neurofibromatosis type 1 (NF1), a common genetic disorder, predisposes patients to the development of both benign and malignant tumors. Although the most common central nervous system (CNS) tumor is a lowgrade pilocytic astrocytoma of the optic pathway, there have been sporadic reports of NF1 patients with more aggressive CNS lesions. We investigated the incidence of aggressive CNS lesions in NF1 patients at our institution. Methods We conducted a retrospective review of all patients with NF1 and any CNS tumor being followed in the Children’s Memorial Hospital NF1 Clinic. Results Seven hundred forty patients with a diagnosis of NF1 were identified. Of these, 145 (20%) patients had CNS tumors, 99 (68%) of whom had optic pathway tumors (OPTs). Five patients (3%) were identified as having highgrade tumors, which consisted of anaplastic medulloblastoma (n=1) and high-grade glioma (n=4). The mean age at diagnosis of NF1 was 2 years. Three of the five patients had a history of an OPT prior to the development of their highgrade lesions. The clinical courses and treatment of these five patients varied. Currently, two patients are alive and receiving therapy at a mean of 10 months following diagnosis.
Conclusion High-grade CNS tumors may occur in children with NF1. Although tumors in NF patients are generally benign, clinicians should have a high index of suspicion of malignancy in patients whose tumors are in an unusual location or behave in an uncharacteristically aggressive manner.
